Health Evaluation
This service delivers a written evaluation of your health status based on your unique health history and current health practices.
The information you provide is gathered through a detailed questionnaire. No discussion with us is involved.
When you order an evaluation, you choose how you receive the questionnaire: by downloading a PDF file that you can complete on your computer or a print copy you can complete manually.
You also have a choice in how the final report is delivered: emailed to you as a PDF or mailed to you in print.
You’ll receive the report in about a week. The questionnaire and the report are based on our research and our health consulting practice.
The Health Evaluation Report is a cost-effective way for you to have us apply our knowledge to your specific health issues.
